Defects

Raise, track, and manage defects across work instructions and work orders to ensure issues are resolved quickly and recorded clearly.

Raising a Defect

In a completed work instruction, click the warning sign icon next to the step you want to raise a defect.

Fill out the defect details, including adding a description of the issue and any rework required, and whether resolution images are required.

By default, the people who completed the step will be the pre-selected assignees. You can change this to assign new team members.

Once raised, assignees will receive a push notification about the defect, they'll also be able to view the defect from the HINDSITE mobile app.


Tracking Defects

Defects are automatically attached to the relevant step in the completed work instruction. Click View details for more information.

On the defect details page, you can update the status, add comments, and add additional assignees.

On the work order, the defect count appears on the associated work instruction.

See all defects across the work order in the Defects tab. Click View to access the defect details page, then navigate to the specific work instruction or step.

Track defects across all work orders from the Defects page accessible via Work orders in the navigation bar.


Responding to a Defect

Click Respond and mark ready, add notes and any supporting evidence, then click Mark Ready.

The defect creator will receive a notification and the defect status updates to In review.

The creator can either request additional rework or approve the resolution. Add commentary if needed.

Once approved, the defect is marked Resolved, and the activity timeline is updated.


Custom Fields

You can configure custom fields to capture additional information on each defect, tailored to your team’s needs. For example, you might add fields for:

  • Defect category or type

  • Root cause

  • Equipment or component affected

  • Estimated downtime or cost impact

To setup custom fields, go to Settings via the cog icon in the top right corner of your screen and select Custom Fields. From there, you can create and manage fields for each status of your defect.
